Die Substanz, in die Nikotin im Körper abgebaut wird. Sie bleibt viel länger nachweisbar als Nikotin selbst (Halbwertszeit etwa 16 Stunden), deshalb messen Labore zur Beurteilung der Tabakbelastung meist Kotinin, nicht Nikotin. Wird auch genutzt, um Passivrauch-Belastung nachzuweisen.
